End of School Year Medication Pick Up
If your student has medication in the health center, please make plans for an adult to pick it up or notify me if you would like for me to dispose of it. (Any medications left beyond the school year will be disposed of) Each school year a new medication administration form is needed if your student will be given medication during school as well as updated care plans. Please let me know if you have any questions or concerns.
For the Upcoming School Year
Emergency Medications at School
If your student has asthma and needs an inhaler, allergies requiring an epipen, or diabetes requiring glucagon or other medications; required paperwork needs to be provided to the school. Also, even if your student is permitted to self-carry, you must still provide backup medication that we will keep in the health center. This is an important safety back-up. Additionally a care plan and Medication Administration Record is to be filled out by the doctor as well as the parent.
If you haven’t received an email from me regarding this and your student needs these forms, please contact me. Fill out each form completely and return to the school as soon as possible. If you need to provide the aforementioned emergency medication, please bring it to the health center in the original container.
